Aftercare and Relapse Prevention Support is a structured, long-term step-down program designed to provide continuous accountability, therapeutic resources, and community connection to graduates of our recovery programs. Our clinical teams collaborate with patients to build detailed, highly personalized relapse prevention plans containing explicit coping protocols, safe-contact directories, and lifestyle outlines. The program grants ongoing access to weekly on-site and virtual group therapy, local sober housing networks, regular vocational support, and an active, vibrant alumni network that organizes social events and community service.
Who Is It Made For?
Completing a rehab program is an incredible achievement, but the real test is applying those skills in daily life. Aftercare is made for:
- Recent treatment graduates: Individuals transitioning out of residential care, PHP, or IOP who want a continuous, structured clinical connection during their first months of independent living.
- Alumni seeking community and accountability: People looking to build healthy relationships with other sober individuals and participate in social, active, and supportive recovery circles.
- Individuals wanting to strengthen their coping strategies: Anyone seeking ongoing professional guidance to manage real-world triggers, career stressors, or relationship changes safely.
- Mentors wishing to give back: Experienced recovery alumni who want to remain involved in the community, offer peer support, and guide newcomers on their path.
